    Check your details

    The doctor confirms your identity and goes through your medical history.

    Examination

    Blood pressure, heart, nervous system and general health checks as set out on the D4 form.

    Eyesight

    Your vision is checked against the DVLA standard, with or without glasses.

    Form completed

    The doctor completes and signs their sections of the D4 for you to send to the DVLA.
